Office Policies

  • Cancellations/No Shows:

    Notify front office 24hours prior to scheduled appt to avoid a $25 no-show fee. Missed appointments represent a cost to us and to other patients who could have been seen at the time set aside for you. We reserve the right to charge for missed or late cancelled appointments.

  • Request for Medical Records:

    Allow up to 4 weeks for preparation and printing, fees may apply

  • Pharmacy refills:

    Fastest way to request a refill of medications is through our patient portal or to call your pharmacy directly and have them request prescriptions from our office. When requesting prescriptions, please inform the pharmacy of the specific medication, dose and quantity needed. If the medication requested was not prescribed by one of our providers, we have the right to refuse your request. Refills will also be declined if you have not seen your provider in the last 12 months.

  • We do not accept workman’s comp or Motor Vehicular Accident (MVA) cases.

  • We reserve the right to refuse service to anyone.
